What To Expect (Job Responsibilities)

• Schedule patients for visit plans to ensure consistency and continuity of care
• Utilize productivity spreadsheets to maintain clinician efficiency and report staffing deficiencies
• Process incoming referrals accurately and strive to achieve timely admissions
• Document updates on patient care and scheduling adjustments in communication notes
• Manage patient visits across all disciplines in accordance with care plans and episode timelines

What Is Required (Qualifications)

• Must be a High School graduate
• Strong attention to detail to ensure accuracy in scheduling and documentation
• Excellent organizational skills to manage multiple tasks efficiently
• Effective verbal and written communication skills for team collaboration
• Strong problem-solving skills to address scheduling challenges in a fast-paced environment

How To Stand Out (Preferred Qualifications)

• Experience in home health care or a related healthcare field
• Familiarity with WellSky EMR or similar electronic medical record systems
• Ability to adapt to changing schedules and priorities in a dynamic work environment
