Role Description

The Clinical Care Coordinator plays a critical role in ensuring that clients feel supported, clinicians are set up for success, and schedules remain full. This individual owns the client journey from first inquiry through ongoing engagement, while actively supporting referral relationships, lead conversion, and service recommendations.This is a performance driven role for someone who understands clinical care, communicates with empathy, and is motivated by measurable impact. This is a 30 hour/week position.

Core Responsibilities

Clarity Calls & Lead Conversion

  • Conduct clarity calls with prospective clients and families to assess needs, urgency, and fit
  • Recommend appropriate services and care pathways based on clinical alignment and client needs
  • Convert leads into scheduled intakes and ongoing care
  • Track conversion rates, follow-up, and outcomes from clarity calls

Client Experience & Care Coordination

  • Serve as a primary point of contact for new and active clients
  • Ensure a seamless onboarding experience from first inquiry through first session and beyond
  • Proactively follow up with clients at risk of disengagement or dropout
  • Maintain a high-touch, compassionate experience aligned with clinical best practices and NourishRX values

Scheduling & Clinician Support

  • Own clinician schedule optimization and utilization
  • Keep schedules full through proactive outreach, waitlist management, and follow-up
  • Support clinicians with client outreach, appointment coordination, and continuity of care
  • Communicate client needs and care gaps to clinicians and leadership

  • Confident recommending the level of care clients need, even when it requires guidance or redirection

Qualifications

  • Registered Dietitian or Licensed Therapist Eating disorder with mental health experience
  • strongly preferred
  • Experience in clinical intake, care coordination, client experience, or healthcare navigation
  • Familiarity with insurance based care models required
  • Experience converting leads in a healthcare or clinical setting strongly preferred
  • Comfortable with EMRs, scheduling platforms, CRMs, and communication tools
