Job Description
Key Responsibilities:
Chart Review & Data Extraction:
Review patient charts to extract clinical data, including diagnoses, medications, treatment regimens, medical procedures, and outcomes.
Ensure accurate and consistent abstraction of relevant data from medical records, adhering to specific protocols and standards.
Identify and highlight key clinical trends or information relevant to research or regulatory needs.
Data Quality Assurance:
Maintain a high level of data accuracy and consistency when transferring information from patient charts to databases or electronic systems.
Address discrepancies or gaps in chart data by collaborating with clinical staff or querying medical records when needed.
Ensure data meets client-specific quality control standards.
Collaboration:
Work closely with project managers, clinical teams, and other stakeholders to ensure seamless data flow and effective communication.
Provide timely updates on progress, potential issues, and outcomes related to chart reviews.
Documentation & Reporting:
Document all chart review processes and maintain organized records of reviewed data.
Assist in generating reports summarizing key findings and insights from chart reviews, as required by the client.
Compliance & Confidentiality:
Adhere to all relevant privacy regulations, including HIPAA and client-specific guidelines, to ensure confidentiality of patient information.
Follow standard operating procedures (SOPs) and clinical guidelines to ensure compliance with industry regulations.
Qualifications:
Education:
Bachelor's degree in Life Sciences, Nursing, Health Information Management, or a related field (or equivalent work experience).
Experience:
Previous experience as a Clinical Abstractor, Medical Records Reviewer, or similar role in a healthcare setting is highly preferred.
Familiarity with chart review processes and clinical trial data management is a plus.
Experience in the pharmaceutical or life sciences industry is a significant advantage.
Skills:
Strong knowledge of medical terminology, clinical workflows, and healthcare data management.
Proficient in using electronic health records (EHR) systems, medical coding systems (ICD-10, CPT), quality databases (e.g., HEDIS, STS) and data abstraction software.
Detail-oriented with excellent organizational skills and the ability to manage multiple tasks simultaneously.
Strong written and verbal communication skills.
Certifications (preferred but not required):
Certified Clinical Research Associate (CCRA), Certified Health Data Analyst (CHDA), or similar certifications are a plus
